At least 24 hours’ notice is requested for cancellations or rescheduling. Cancellations made with less than 24 hours’ notice and missed appointments may be charged the full scheduled service fee. Please contact Michelle as soon as possible when an unexpected situation occurs.

Arriving late may shorten the available service time so the next client is not delayed. The full scheduled service fee still applies.

If you are feeling unwell or may be contagious, please contact Michelle and reschedule. Illness-related situations are handled thoughtfully and individually.

Payment is due at the time of service unless a prepaid package, gift certificate or other approved arrangement applies. Accepted payment methods are displayed during booking or discussed before the appointment.

Gratuity is appreciated but never required.

Please share any changes to your health, medications, recent procedures, surgeries or pregnancy at the time of booking so your session can be planned appropriately.

Depending on how you interact with the practice, information may include:

• Name and contact information
• Appointment and communication history
• Intake information and information you voluntarily supply about health, medications, sensitivities, pregnancy, procedures or service preferences
• Transaction and purchase records
• Website form submissions
• Technical website information such as browser, device, cookie or general usage data

Information may be used to:

• Schedule and manage appointments
• Personalize services and help evaluate suitability
• Process payments and purchases
• Communicate about appointments, questions or requested services
• Maintain appropriate business and transaction records

The practice uses trusted third-party service providers for booking, intake forms, communications and payment processing. Online scheduling and intake are handled through Jane, and online purchases such as gift certificates and prepaid packages are processed through Square.

These providers receive only the information reasonably necessary to perform their services and handle information under their own privacy and security practices. Michelle does not control another company’s systems or privacy practices.

This website may use hosting, security and analytics tools to understand general website use and maintain site performance. These tools may process limited technical information such as pages visited, browser or device type, referring page, general usage activity and IP-derived approximate location. Functional cookies or browser storage may also be used for features such as client-account sessions. The website does not currently use advertising cookies for targeted advertising. If website tools or practices materially change, this policy may be updated.

Relax & Radiate, PLLC does not sell client personal information. Information may be shared with service providers when reasonably necessary for scheduling, payment processing, website operation, communications, order fulfillment, legal compliance or safety. Information may also be disclosed when required by law or with the client’s authorization.

Client intake and service information is treated as confidential and is protected using reasonable administrative and technical safeguards, including limited access and secure service providers. No method of storage or transmission can be guaranteed to be completely secure, so absolute security cannot be promised.

Information is retained for as long as reasonably necessary for the purpose for which it was collected and to meet applicable professional, accounting, legal and business requirements.

Some information cannot be deleted when it must be retained for legitimate legal, accounting or professional reasons. Michelle will explain what can and cannot be changed in your specific situation.

This website is not directed to children under 13. For services involving a minor, a parent or legal guardian should manage communications and provide consent.

Massage, bodywork, facials, lash services, vibroacoustic support and other offerings are wellness or cosmetic services. They are not medical diagnosis or treatment and do not replace care from an appropriate licensed healthcare provider.

Individual experiences and results vary. No particular result, duration of result or outcome is guaranteed.

Clients are responsible for providing accurate and current information relevant to service safety and suitability, including medications, allergies, sensitivities, pregnancy, recent procedures, surgeries, implanted devices and health changes.

Michelle may modify, postpone, decline or end a service when safety, suitability, professional scope, inappropriate conduct or other reasonable concerns arise.

A complimentary video consultation is requested for all first-time clients. It is a friendly conversation to discuss your goals, answer questions and help determine which service may be a good fit. It is not a medical evaluation or diagnosis, and it does not guarantee acceptance as a client. If Michelle’s services are not aligned with what you need, she is glad to suggest other professionals to consider.

Prices, service descriptions, availability and scheduling may change. The price displayed or confirmed at the time of booking or purchase applies, unless an obvious error occurred.

Acceptance of an HSA or FSA card does not guarantee that a service qualifies for reimbursement. Clients are responsible for confirming eligibility with their plan administrator.

Dollar-value gift certificates do not expire and may be used toward any service. Gift certificates are non-refundable and are not redeemable for cash.

Packages are purchased prepaid and are valid for 12 months from the date of purchase. Sessions are tracked in your client account.

• Scheduling remains subject to availability, and a package purchase does not guarantee a particular appointment date or time.
• Cancellation and missed-appointment policies still apply to sessions booked with a package.
• Prepaid packages are non-refundable once purchased, except where required by law. If a medical or exceptional circumstance prevents continued use, contact Michelle to discuss available options.
• Packages are intended for use by the purchaser and may not be shared or transferred without Michelle’s prior approval. Any exception is considered individually and is not guaranteed.
• Package sessions are intended for the services listed in the package. With Michelle’s prior approval, a session may be upgraded to a longer or higher-priced eligible service by paying the applicable price difference. Upgrades are subject to suitability, scheduling and availability.
• The price difference is based on the current regular prices of the included service and the upgraded service at the time of the appointment. Package sessions have no cash value and cannot be exchanged for cash, converted into account credit, applied to lash services or used toward one-time Combination Services.
• Choosing a shorter or lower-priced service does not create a refund, credit or additional session.
• An upgrade does not extend the package expiration date.
• Michelle’s approval is required before the upgraded appointment is scheduled.
• Upgrades are not guaranteed.

Sessions are for therapeutic, wellness and cosmetic purposes only. Sexual, threatening, discriminatory, abusive or otherwise inappropriate conduct may result in immediate termination of the service, with the full service fee charged where permitted.

This website may link to independent services such as booking, payment or product-provider websites. Those third parties operate under their own terms and privacy practices, and Michelle is not responsible for their content or policies.
